My plastic surgeon told me that the results of Restylane fillings in my holes under my eyes would last more than two years without touching. I asked because I've never seen that length announced anywhere. He said it would last so long because I rarely moved that area so that the filling would break down slowly. Although I trust him and I know he does these injections all the time, I'm looking for a second opinion. I don't want stuff if it'll only last 6 months, but if it's two years then I sell myself. I've been injecting fillings for more than 13 years and one thing that's true is that there's variability as soon as the fillings will last in any area and for any patient. The area of the eye is an area where the fillings last longer. Fillers in this area "can" last for more than 2 years, but there is no guarantee in that. (Unless the injection doctor wants to guarantee it). I wouldn't feel comfortable making that guarantee. In the low-eyed area (infraorbital or tear traction), no filler is approved by the FDA to treat this area, so we cannot say with 100% certainty how long a given filling will last based on a well-controlled trial. We have to trust our experience. That said, what I say to patients is: 1) We have to get the correct level of correction. This could mean more than 1 treatment to get you to complete correction. More often, I will treat a patient with 1-2 filler syringes and then re-evaluate 1 month. Some patients will need more a month. Sometimes, I'm going to layer different fillers on top of the first treatment. (2) Fill adjacent areas to create a final result balanced and pleasant aesthetically. Many times, the cheek should be corrected to help support the area below. Treating the cheek can go a long way to correct the area below the eye, but often, a true treatment under the eye is needed for an ideal correction. 3)What filler is used: Some fillers last longer than others. Vollure may last the longest but some patients have had prolonged or delayed inflammation of this product. I have not personally experienced this, but it has been noticed by others. So to summarize, yes, fillings in the lower trough region or tear can last up to 2 years, but this is very variable. I prefer to treat correction, re-evaluate 1 month and fill more as needed and then follow the patient. This minimizes excessive correction and allows the most natural look as the low eye is one of the easiest areas to be corrected. Then the patient is treated as necessary.
